These consumers have highly complex regimens, averaging 10 medications daily with inconsistent fill patterns that require monthly adjustments in dosage synchronization.

Through its Adhere™ platform, PharmMD reviews the medication list with a consumer each month, synchronizes any pharmacy changes with their providers and couriers the medications to the consumer’s home, resulting in an average of 1.39 medication deliveries per month. Multi-Dose Adherence Packaging Pharmacy

Through a strategic partnership with a national full-service pharmacy, PharmMD delivers a better, simpler experience for polypharmacy consumers who are challenged by social determinants, such as poor access to care and poor health literacy. The partner is fully integrated with PharmMD’s Adhere™ platform with a real-time platform integration.

PharmMD’s partnership with the national full-service pharmacy enables it to serve consumers who take multiple medications, typically 4.5 per day on average. Each month, consumers receive a personalized roll of pre-sorted medications, along with a convenient dispenser and any other medications that cannot be placed into packets, such as liquids and inhalers. Every shipment includes a medication label that has a picture of each pill as well as notes on how it should be taken. The solution uses a proprietary software platform to help manage each consumer’s medications, coordinate refills and renewals and make sure each shipment is sent on time, every time. Real-time notifications are provided using an online dashboard, which also lets consumers control their shipments, refills and copays.
