Medication Reconciliation

Medication reconciliation is a labor-intensive process in which a patient’s list of discharge medications is reconciled against the list of medications obtained at time of admission. If the process is patient-centered, it can be effective in preventing medical errors and adverse drug interactions after discharge. A pharmacist’s MTM program improves medication reconciliation and prevents hospital re-admissions. The involvement of pharmacists provides a patient with better understanding of their medical condition and the medication therapy required for recovery.
