PharmMD works with physicians in two different ways:
- Prescribing Physicians may receive information from PharmMD about one of their patients.
- Physician Groups (IPAs) can utilize PharmMD’s Medication Therapy Management (MTM) services to help gain a deeper, more comprehensive picture of their patient’s health and healthcare usage.
How PharmMD Helps Prescribing Physicians
Individual patients may see different doctors or receive medical treatments and prescription medications outside of their primary care doctor’s supervision. If no one person is reviewing a patient’s entire medical profile, there is the potential for dangerous medication interactions.
PharmMD works with companies, health plans and physician groups to consolidate a patient’s medical and pharmacy claims information so that PharmMD’s proprietary MTM screening system can review the entire record.
If there are potential medication-related issues, PharmMD’s trained team of pharmacists personally review the record and determine if the patient’s primary care physician should be notified. If notified, each physician can determine if they need to take action with the patient and recommend any changes in a patient’s overall medication or care plan.
How PharmMD Helps Physician Groups (IPAs)
PharmMD’s comprehensive Medication Therapy Management (MTM) services utilizes evidence-based medicine, national guideline standards and proprietary rule sets to identify potential improvements in medication management. Our network of pharmacists review patients’ medical information and make recommendations to improve the outcome for each individual patient.
PharmMD’s goal is to ensure that the care a patients receives is delivered in the most appropriate manner to achieve optimal therapeutic outcomes. When PharmMD identifies a potential medical intervention or area for improvement, one of our pharmacists will communicate our findings to the patient’s physician.
