PharmMD is a free service to patients provided through your employer or health plan to help you with your medications. PharmMD collects information from your physicians, pharmacists and you to identify areas for improvement. We do not share your individual health information and/or conditions with your employer.
PharmMD’s services involve no change to your current prescription drug benefits.
PharmMD’s network of pharmacists will review your medical information to answer three questions:
- Are you on the best medications for your medical conditions?
- Are the medications helping achieve the desired results?
- Are you on medications that can be changed or discontinued to save you money?
Please note: PharmMD only makes recommendations based on what we know about your medical record. We do not make changes to your medications. We also do not keep you from getting access to medications or medical services.
In addition to the benefits of identifying better medications, eliminating potential negative medication interactions, and improving the care delivered to you, PharmMD helps you to become a better health care consumer.
Included in PharmMD’s service is the creation of a Personal Medical Record (PMR) where you will find all your medications listed in one place. You can share this PMR with anyone providing health care services to you to give them a full picture of your medication use so they can provide better treatment. You can also add additional information regarding any vitamins or herbal supplements you may be taking so your care providers can better care for you. Based on the information in your PMR, PharmMD also provides you with a Medication Action Plan (MAP) that identifies ways you may use your medications more effectively.
